# Intake forms overview

> Collect client and prospect information with AI-built forms that map answers directly into Slant.

Intake forms are online questionnaires you build in Slant and send to clients and prospects to collect their information — personal details, financial data, documents, and signatures — without back-and-forth emails or paper forms.

Answers map directly to Slant fields, so when a form comes back you review the responses and apply them to the household record in a few clicks.

## How intake forms work

1. **Build a form** — describe the form you want and Slant's AI builds it, or start from a template. Every form uses one of four polished layouts with your firm's branding. See [Creating and editing forms](/platform/intake-forms/creating-and-editing-forms).
2. **Publish it** — publishing creates a numbered version that recipients see. You keep editing the draft without affecting anyone mid-form. See [Publishing and sharing](/platform/intake-forms/publishing-and-sharing).
3. **Share it** — send a personal invitation to a client or prospect, or turn on a public link anyone can open. See [Sending invitations](/platform/intake-forms/sending-invitations).
4. **Review submissions** — completed forms land in your submissions inbox, where you compare answers against what's on file and apply the changes. See [Reviewing submissions](/platform/intake-forms/reviewing-submissions).

## The Intake forms page

Open **Intake forms** in the sidebar under **Platform**. The page has three tabs:

* **Forms** — every form in your book, grouped by status: **Building** (the AI is still drafting it), **Draft** (not yet published), and **Live** (published). Each row shows the form's sections, invitations, and submissions. Filter by number of invitations or submissions, and archive forms you no longer need.
* **Invitations** — every invitation you've sent, with its status (**Open**, **Submitted**, **Revoked**, or **Expired**), the invitation link, and when it was created and submitted.
* **Submissions** — completed responses from invitations and public links, flagged **Needs review** until you apply or dismiss them.

Clicking a form opens its detail page: response stats (invitations, opened, submissions, needs review), the list of invitations, the public link toggle, version history, and a live preview. From here you can **Edit form** or **Send invitation**.

## Where intake forms appear

* **Intake forms page** — build, publish, and track everything from **Intake forms** in the sidebar under **Platform**.
* **Household pages** — send a form to a client or prospect from the **⋮** menu on their page, and see their invitations and submissions in the **Intake forms** section of their **Records** tab.
* **Chat** — ask [Book chat](/dashboard/book-chat) or a [household's chat](/records/households/chat) to send a form for you.

## What forms can collect

Forms support the field types advisors need for onboarding and data gathering:

* **Contact and personal details** — names, dates, emails, phone numbers, and addresses that map to Slant fields
* **Financial details** — currency, percentage, and number fields, plus multiple choice and rating questions
* **Document uploads** — collect tax returns, statements, and IDs; files are delivered to the household's folder in your connected [file storage](/setting-up-slant/integrations/file-storage)
* **Acknowledgments** — have the recipient type their name to acknowledge disclosures
* **Meeting scheduling** — embed your scheduling link so recipients book a meeting inside the form
* **Repeating groups** — let recipients add multiple entries, like dependents or accounts

Fields can be required, show conditionally based on earlier answers, and come prefilled with what Slant already knows when you send a personal invitation.

## Automatic reminders

Invitations sent from the Slant email address can send automatic follow-up reminders on business days until the recipient submits the form. You control the spacing and the maximum number of reminders per invitation, and reminders stop on their own once the form is submitted or the invitation is revoked. ## Security

* Personal invitation links are unique per recipient. Before opening the form, the recipient verifies their identity with a code sent to their email. See [What clients see](/platform/intake-forms/what-clients-see).
* Public links are open to anyone who has the link. You can turn a public link off at any time, and archiving a form closes its link.
* Recipients' progress saves on their device, so they can leave and finish later.

## Permissions

Any user with access to the book can create, edit, publish, and archive forms, send and revoke invitations, and review and apply submissions.

## FAQ

    No. Describe the form you want — or attach an existing form as a PDF, Word document, or image — and the AI builds it for you. You refine it by chatting, not by dragging fields.

  <Accordion title="Can recipients fill out a form on their phone?">
    Yes. Every layout works on desktop and mobile, and the editor includes a mobile preview so you can check before sending.
  </Accordion>

  <Accordion title="What happens if I edit a form someone is filling out?">
    Nothing changes for them until you publish. Personal invitations stay on the version that was current when they were sent, and public link respondents mid-form have a grace period to finish on the version they started.

  <Accordion title="Can I delete a form?">
    Forms are archived rather than deleted. Archiving stops new invitations and closes the public link, but keeps existing invitations and submissions.
